November 1st - December 15th (for January 1st effective date)
December 15th - January 31st (for March 1st effective date)
Special enrollment periods (SEPs) are given throughout the year when requirements are met

Approximate yearly income
Employer's name and phone number
Email address (if you don't have one, we will make one for you)
Previous insurance information (company, policy number, effective date, and cancel date)
